Sensitive skin is not one problem, which is why generic advice for it so often fails. When researchers examine people who describe their own skin as sensitive, they keep finding at least three different mechanisms underneath one complaint. Some people have sensory nerves that fire at the slightest provocation. Some have a weakened barrier that lets irritants straight through. And some have blood vessels that dilate at almost nothing, while their barrier measures as entirely intact.
That third group is the reason this distinction matters commercially as well as clinically. The standard prescription, ceramides, drop all your actives, wait it out, is aimed squarely at the second group. If what you actually have is vascular reactivity, you can follow that advice perfectly for three months and still be red.

What are the three types of sensitive skin?
Worth saying plainly: "sensitive skin" is a lay term rather than a medical diagnosis. Population surveys consistently find that a large share of adults, women more often than men, describe their own skin as sensitive. The label describes an experience, not a mechanism.
1. Sensory reactivity: it stings, but nothing shows
This is discomfort without visible change. Products sting, prickle or burn on application, sometimes within seconds, and when you look in the mirror there is nothing to see. The classic triggers are lactic acid and propylene glycol, and low-pH vitamin C serums are a common real-world culprit.
The mechanism is sensory nerve overreactivity: nerve endings responding to stimuli that would not register for most people. Clinics assess it with a lactic acid sting test, essentially a formalised version of what you already do to yourself every time you try a new serum.
Your tell: it hurts, but a photograph would show nothing.
2. Barrier-driven reactivity: tight, flaky, everything burns
Here the outer layer is genuinely compromised, so more of what you apply gets where it should not, and more water escapes than it should. The pattern is recognisable: skin feels tight within a minute of cleansing, moisturiser stings on application and then settles, there is flaking around the nose and mouth, and products you used happily last year now sting.
This is the type that is most often acquired rather than inherited. Over-exfoliation, a too-fast retinoid ramp, a foaming cleanser that is too strong, hard water, a cold snap. It is also the type that genuinely does resolve with the standard advice, usually over several weeks.
Your tell: the tightness. Barrier-driven skin feels tight; sensory-driven skin feels stingy.
3. Vascular reactivity: it goes red and stays red
Flushing in response to heat, cold wind, exercise, alcohol, spicy food or stress, with redness that outlasts the trigger by a long way. Research into self-described sensitive skin has repeatedly turned up a subgroup with markedly heightened vascular reactivity but no measurable barrier impairment. These are people whose skin is, by the objective measures, not damaged at all. It just goes red.
Your tell: a trigger you could name, the shower, the gym, the glass of red wine, rather than a product.
Plenty of people have two of these, or all three. The point is not to box yourself in but to work out which is currently doing the most damage, because that is the one to address first.
Use the clock: how fast it reacts tells you what happened
This is the single most useful diagnostic you have, and almost nobody uses it. The timing of a reaction narrows down its mechanism more reliably than the ingredient list does.
- Seconds to a few minutes, no visible change. Sensory irritation. Uncomfortable, generally not harmful, and often reduces as skin becomes less reactive overall. Not a reason to bin the product, but a reason to apply it to dry skin rather than damp, and buffer it under moisturiser.
- Twenty minutes to a few hours, with visible redness or burning that fades over a day or two. An irritant reaction. These are dose-dependent: the same product at a lower frequency, a smaller amount, or on a less compromised barrier may cause nothing at all.
- One to four days later, an itchy rash that spreads beyond where you applied it. This is the pattern of delayed contact allergy, and it behaves completely differently. Clinical patch testing works on exactly this timescale. Patches are removed at 48 hours and the skin is inspected 48 hours after that, because reactions usually take 48 to 96 hours to develop. Delayed reactions are not something to self-manage by swapping moisturisers; they are worth a GP or dermatologist conversation, because identifying the specific allergen is the only thing that reliably fixes them.
The same logic applies to persistent facial redness with visible vessels or bumps. That is a medical picture rather than a skincare one, and it is worth having assessed properly before you spend another six months on products.
What changes depending on which sensitive skin type you have?
If you are sensory-dominant
Your enemy is concentration and contact time, not ingredient categories. You can usually keep the actives you want; you just cannot apply them the way the box says. Apply to fully dry skin, because damp skin increases penetration and increases the sting. Buffer: moisturiser first, then the active on top, for the first month. And be suspicious of solvents rather than the headline active; propylene glycol and high levels of denatured alcohol are frequent offenders in serums that "shouldn't" sting.
Exfoliation is worth reconsidering rather than abandoning. Polyhydroxy acids are larger molecules than glycolic acid and work more slowly at the surface, which is why PHA formulas are the usual starting point for people who cannot tolerate a standard AHA. If you are barrier-dominant
This is where the conventional advice is right, and needs following properly rather than half-heartedly. Two changes carry most of the weight.
First, the cleanser. A cleanser combines the highest surfactant load in your routine with the most whole-face contact, so swapping a high-foaming wash for a cream, milk or oil-based one changes how skin feels within days. Browse gentle cleansers and read the surfactant, not the marketing.
Second, lipids. Ceramides, cholesterol and fatty acids are the material the outer layer is built from, and topping them up topically is the reason ceramide-containing moisturisers make skin feel less tight and look less flushed. Niacinamide is worth adding alongside. It is unusually well tolerated and pairs comfortably with almost everything.
Give it four to eight weeks before you judge it, and resist reintroducing actives the moment things improve. That is the most common way people end up back at the start.
If you are vascular-dominant
Barrier care still helps, but it is not the lever. Trigger management is. Keep a fortnight's note of what precedes a flush. You are looking for patterns around temperature swings, hot showers, exercise, alcohol and stress, not products.
On the topical side, the ingredients with the most credible reputation for the appearance of redness and uneven tone are azelaic acid and niacinamide. Azelaic acid formulas are generally introduced two or three times a week rather than nightly, because tolerance builds gradually.
Daily sunscreen matters more for this group than for any other, because UV exposure is a well-established provoker of facial redness. Wearing it every day also helps prevent cumulative UV damage. Mineral filters using zinc oxide or titanium dioxide are often better tolerated by reactive skin, which is why mineral sunscreens tend to be the default recommendation here. How do you patch test at home so it tells you something?
Most people patch test badly, then conclude patch testing does not work. Three fixes:
- Test where you will use it. Facial skin is thinner and more reactive than the inner forearm. A jawline or behind-the-ear patch predicts your face far better.
- Wait four days, not twenty-four hours. Irritant reactions show up quickly; delayed reactions take one to four days. A 24-hour test only catches half the problem.
- Test one thing at a time. If you introduce three products in a week and react, you have learned nothing and now have to eliminate all three.
How do you reintroduce actives without triggering sensitivity again?
Once things have settled, the failure mode is almost always speed rather than choice of product. A workable ladder: one new product at a time, two nights a week, on an otherwise unchanged routine, for two weeks before increasing frequency, and four weeks at full frequency before introducing anything else. What does "for sensitive skin" on a label actually tell you?
Less than you would hope. The phrase is not regulated, so it signals intent rather than evidence. What is worth checking instead: fragrance and essential oils near the top of the list, denatured alcohol high up in a leave-on product, and whether the brand publishes a full INCI list at all. Brand philosophies genuinely differ rather than one being better: some sensitive-skin ranges are built around minimal ingredient counts, others around delivering conventional actives in encapsulated or buffered form so reactive skin can still use them. Which suits you depends on which of the three patterns you recognise in yourself.
Where to start
Work out which pattern dominates, change one thing, and give it four weeks. If your skin stings without showing anything, work on how you apply. If it feels tight, work on cleanser and lipids. If it flushes and stays flushed, work on triggers and sun protection, and get persistent redness looked at properly rather than treated as a product problem.
